THYROMETER

- Critical to your effectiveness as a clinician is your ability to obtain objective information. Your "intelligence" will get you and your patients into trouble. Regardless of how smart you think you are,as a human,you are susceptible to making emotional judgements. The more I started examining patients for evidence of thyroid dysfunction,the more I realized this. My "intelligence" from medical school dictated that if a patient's TSH was normal,the patient was euthyroid. However,the more I used objective measurements of thyroid function,the more I understood that most of us were practicing more religion than science. Temperature assessment is one way to assess thyroid function. Human cells like a temp of around 98.6F-head to toes. This applies to all human cells,not just the ones under the tongue. As the ambient temperature declines,its an indication that there is dysfunction in heat/energy production by the cells or there is a compromise in circulation. In either case,this is when things start to go downhill. Its important to assess not only the ambient temperature and ability of cells to produce heat/energy under the tongue,but as many places as possible. One tool I use to do this is a remote laser thermometer. This trick was taught to me by my good friend Merle Jane M.D. You can use this device to assess skin temperatures just about anywhere. I check toe temps and compare them to the sublingual temp. The other device I use I call a thyrometer. It has other names in the sex shops.......so I've been told. This device works basically on the same principle as a thermometer. The speed at which the volatile liquid enclosed in the flask will rise and "boil" is contingent on the holders ability to generate heat. Normally,this should take 3-5 seconds. 57 seconds,amalgamated with the rest of my exam, tells me there is likely a problem. I don't depend on any one parameter to make a diagnosis. Oral temps are often deceptive due to chronic sinusitis or beverages. Tight shoes can compromise circulation. Point is to build up a solid clinical case to rely on rather than blood tests. If you get one,you'll need to calibrate it with people you know to have good thyroid function. They are quite fragile,so keep away from grand kids.
